Why are we here?

Community Balance Partners was founded with a vision to ensure that senior communities, those at higher risk of falls, were able to get actionable, objective, effective balance screenings.

We saw a gap in the healthcare system—where preventative care fails to truly address one of the most pressing health and wellness issues: falls.

It is estimated that the financial toll of falls will top $100 billion by 2030. Aside from the tremendous financial burden this is on our healthcare system, they can also have a major negative impact on the lives of seniors.

how do we work?

schedule & Advertise

Book a 2-hour session directly with us, and use our F.R.E.E. flyers to advertise the program to your residents. We’ll send out a Community Balance representative trained in the SenSen Balance Assessment Tool and equipped to explain how the program will work and run the whole thing—no additional support required.

host

Our FREE (Fall Risk Evaluation & Education) program consists of a brief overview, individual screenings where we’ll spend 5-8 minutes per person on evaluations. Residents will walk away with a Fall Risk Report Card which they can keep and share with their Physical Therapist or PCP.

REPEAT THE PROGRAM

The F.R.E.E. program can have maximum and lasting impact when repeated consistently—we recommend quarterly. This give residents a chance to improve balance and reduce fall risk propensity, and demonstrate that their efforts are having a noticeable and measurable impact on their health.

RESIDENT BENEFITS

Enhanced Safety and Prevention

One of the primary benefits of the F.R.E.E. Program is the enhanced safety it provides. By undergoing regular balance assessments, seniors can identify potential issues before they lead to falls. Our comprehensive Fall Risk Report Card highlights specific areas where they may need to improve their balance, helping them take proactive steps to reduce their risk. This preventative approach not only protects seniors from physical injuries but also helps them maintain their independence and avoid long-term health complications associated with falls.

Informed Health Decisions

Our balance assessment tool provides seniors with objective, evidence-based information about their balance and fall risk. This valuable insight enables them to make informed decisions about their health. With a detailed Fall Risk Report Card in hand, seniors can discuss their balance status with their primary care physician or physical therapist, ensuring that they receive personalized interventions and support. This collaboration between seniors and healthcare providers fosters a comprehensive approach to health management, leading to better outcomes and improved quality of life.

PEACE OF MIND AND CONFIDENCE

Knowing that they have a clear understanding of their balance propensity gives seniors peace of mind. Our program empowers them with the knowledge they need to take control of their health, which in turn boosts their confidence. Seniors who are aware of their fall risk and have a plan in place to address it feel more secure in their daily activities. This increased confidence allows them to participate more fully in their communities and enjoy a higher quality of life, free from the constant worry of falling.
